Cannot get files named meaningfully

When I start a download with version 4 (up to & including the latest), I get two files recorded, with neither having a sensible name. I have tried all naming settings but still never get the proper name.

1) How can I stop the program from saving two files, and

2) How do I get the correct name?

Here is an example URL (slightly altered ;-), where the correct name - "Finalizing Editing" - is shown in Firefox as the Title:
